Yes, it's both. The asker has the option of choosing the best answer him-/herself, or asking others to vote for the best answer. If the asker doesn't exercise either option, the question goes to an open vote after a while.

The way that it appears to me is if the person that asked the question picks you as the best answer then you get the points...If the person who asks the question doesn't pick a "best answer" then the level two or higher people can vote on the best answer after a period of time.

The percentage is based on how many questions you have answered compared to how many were chosen as best. Simply numbers configuration.

out of all the answers you've given to questions, sometimes people liked yours best, gave you the ten points. but it's just a percentage of total answers given by you ...i.e: 100 total answers, 10 chosen best answer, =10%

I do believe it is the % of the BEST chosen ones... you know when you ask a question, you choose one that is the best.. that one... not the little votes any random person can check.. .;)
