MC is an anagram for a latin phrase which tell you that the person has masters of surgical medicine.
Anyway in an early episode of season one Christine and Meredith are talking in the lobby of the hospital about how tough their days were. Meredith was complaining that she kissed Derek in the elevator and she doesn't seem to be able to avoid him, and she is having to go everywhere carrying a severed penis in an ice chest. To this Christine in one of her shippy moods responded "Oh, I wish worst problem was having to deal with Dr. McDreamy." Later in the Same season, when Izzy is upset at Meredith feeling she is getting all the breaks in getting good surgeries she too brings up the nickname Dr. Mc Dreamy to talk about Derek. Needless to say the name stuck among the interns.
In season two, his friend mark came in and it was learned that he was a plastic surgeon, who had slept with Addison, Derek's wife, and was now chatting up Meredith, when Derek punched him. Izzy, Christina and Meredith debated over the nickname and Mc Steamy took the nod, over Mc Sexy.
Next week will see the entrance of Dr. Mc Hotty, the first female to get such a nickname.

There is no actual written history behind the name "Dr. McDreamy," it's just a cute catchphrase that led to McLabeling being a trademark of Grey's Anatomy (McVet, McLife, McDog, McVomit, and McSteamy are the ones I can think of)
McSteamy is a nickname for Mark Sloan, the plastic surgeon who is the newest addition to the regular Grey's Anatomy cast. He is Derek's former best friend, and also the man that Addison slept with, which Derek caught them doing and left her for Seattle.
The name "McSteamy" was given to Mark the first time he appeared on the show, which was when he showed up randomly to try and bring Addison back home to NY. He starts flirting with Meredith, and Derek knocks him to the ground with a very impressive punch. Anyways, Meredith, along with the other female interns, label him as "McSteamy."

Dr. Sheppard got his nickname "McDreamy" because he is the man of Merideth's dreams. McSteamy is Dr. Sheppard's ex-best friend, who also slept with Addison Sheppard, causing Dr. Sheppard to leave New York for Seattle where he met Merideth. McVet is Finn who is/was Meredith's other boyfriend. For some reason Meredith likes to add "Mc" to the beginning of everything. Soon, according to the preview there will be a McHottie.

They just made that name up somehow. I guess because he was the man of meridaths dreams. Mcsteamy is mark. He was mcdreamy's best friend until he caught him in bed with his wife.

I am thoroughly hooked on Grey's Anatomy. My quality pal IS Meredith Grey! She dated a sizzling dr final 12 months after which learned he used to be married. So, it turns out Mark has been staying not too far from the hospital… At a really posh hotel. On the 22nd floor. With Addison! She apparently summoned McSteamy to her hotel room after she left the bar the other night. Seriously. Mark flew all the way out here for a little fun on the 22nd floor? Dude. That’s a long way for a booty call.
A booty call? Mark doesn’t necessarily see it that way. At least, he doesn’t want to see it that way. Nope. You see, McSteamy thinks he has a real chance with Addison. He thinks he has a real chance of bringing Addison home to New York. After all, there’s nothing keeping Addison in Seattle any longer, right?
Maybe. Maybe not. I’ll admit it. I know I was a little harsh on Addison when I first met her. But, I’ve grown to love her. I’d seriously miss seeing that little firecracker in my bar from time to time. She’s hilarious. She’s fun. She’s smart. She’s hot. You know, it’s really not too hard to figure out why Mark wants Addison to go back to New York with him…

Because they think he's dreamy and they're just putting a "Mc" in front of it. Mc steamy is the guy his wife cheated on him with (his ex-best friend) and they call him that because they think he's hot and the first time one of them saw him was when he had come out of the shower wearing a towel (shower, hot, steam, get it?).
