rarely (but with increasing occurence), it will never fill up, and stay low and keep running (even when the valve is apparently shut).

Usually, it just drains REALLY slowly into the tank until it is full an hour after each flush.

Replace the fill valve. Make sure the water valve located on the left side of toilet is on all the way. Depending on the type of fill valve you have, Be just as easy to install a repair kit for it

First, is the water pressure in the rest of the house okay? If so, try opening the valve that sticks out of the wall behind the toilet a little bit more. If that doesn't work, you might need a new toilet fill valve. They are inexpensive, and not very hard to replace. Just follow the instructions on the box.

shut off water to toilet / empty tank water and put a towel in to dry out first / then disconnect hose to tank ,or from valve pending connection condition
check if clogged by blowing threw it or just eye ball it if disconnected 100% i/ then check connectors for h2o at valve and in tank for blockage. But I think its problem is its Valve is not opened all the way for slow fill.

BUT Are you loosing water at drain?

I would say that if you still have the ball valve you should replace it, that is nothing but trouble, besides, the new Fluidmaster valve (excellent brand), cost around $10 dollars.
Good luck!

If that doesn't work, see if you need a new rubber/plastic ball attachment for the inside of your toilet bowl tank.
