It was an accident.

Actually it is believed that someone in Germany ground loose scraps of meat together and sold it for less money. It was very low quality so they had to add good meat to it for their own good.
% lean probably saves your life.
You can grind any cut of beef such as ground chuck or sirloin burgers.

The hamburger was first developed by butchers in Hamburg Germany (hence the name). Not sure exactly when, but it was some time ago. Germans made a lot of sausage using various recipes, and hamburger was simply another variation on a sausage recipe and cooked, as many sausages are, in a patty form without stuffing it into a sausage skin first.
