Why are Republicans saying that the Democrats are exploiting Michael J. Fox (who chose to do the commercial) and calling the whole thing foul, but it was okay for them to expliot Terry Shivo (who had no say in the matter) and run her family through the wringer?

Because MJFox doesn't represent their point of view.
Republicans don't like anyone that doesn't support their viewpoints, and opposes them politically. Since Terry Shivo couldn't oppose them, they "took on her cause" (and prolonged much suffering for all involved).

definitely exploitation on the grounds that fox was a huge big actor since the beginning of back to the future !
