Because of government policies. From a casual observation, farmers are the least paid people in a country, but the most important. Add to that they are the least educated. Now add a government which regards starving people as an advantageous commodity. From a dictatorship point of view there are several uses in having starving people.
First they can get foreign aid for free / very cheap and then sell it / unfairly distribute it. North Korea isn't the only country currently doing this.
Secondly it provides them with a ready supply of volunteers for the army. By joining the army a person has guaranteed food and pay. You may also get free / very cheap accommodation / clothes / medical / education for your wife and children.
I heard once that no secret ballet democratically elected government had a famine in the 20th Century, I'm not sure if this is true, but if it is, it shows even a minimal amount of accountability does influence a government.

Anyway, how "come"? Well, basically, as far as we know (and very little concrete info comes out of NK) the communist dicatorship of Kim Jong-Il is a mess and can't run the country worth a nuclear missile.
What's wild is go check out a LANDSAT pic of that part of the wortld taken at night. Compare South and North Korea--the North is almost pitch black. The South were willing to hook up the North to their electric grid but, get this, despite the humanitarian (and probably a few won/favors) offer, the North's grid is so outdated it won't work even if they did it. The place is freakin' backward.

Because they are being forced to participate in a global economy created by "core" or "developed" nations. These core areas use the economic failure of places like North Korea to exploit and take advantage through inequitable trade relationships.

Because the government there invests money in weapons rather than in the people. That's why charity money sometimes get stolen too.
