Just be honest with the employer as to why you want to leave. If they think you are a good employee and can accommodate you on a different shift they will. If they cannot accommodate you, then at least you tried.
You are leaving for good reasons. Don't let that distract you or make you hesitant in doing it. A good employer/manager will understand.
I would do it in person if you can, but if you feel you need to do it over the phone, to give them a heads up for scheduling, etc., then go ahead. Just don't do it over the phone because you are afraid of facing them. Have the maturity to do it in person.

Generally, a formal 2 week notice should be given in writing to your supervisor. Over the phone makes it seem like youreally don't care, and if things at your new job donot work out, you may consider returning. I would definaltely give a resignation letter. The letter would also protect you in the event that someone says you did not give notice....

Definitely do it in writing. Not only does it cover you in the future, as there is a record of you resigning, but I also think it is better business practise to not do things like that over the phone. I actually am not sure if an employer would accept a verbal resignation anyway.

I think that it is best that you proceed to give your two weeks notice in writing, simply because it more professional than calling over the phone. This way if you find another job that you are interested in, this will not affect the decision by the employer.

Do it in person, remember, you still have to work the two weeks after you give the notice. You do not want to rub others the wrong way and shirk your responsibility of a face-to-face notice.

Won't you have to work sometime in the next two weeks? It doesn't really make sense to do it over the phone unless you were going to straight out quit.

Two weeks notice should always be in writing or by mouth directly in front of the employers face. (If you want your work history to remain clean that is)
