Well, there are two parts to this question. One is the presentation layer and the other one is the backend to actually do the payment process.
The presentation layer can be done in HTML.
The above is a simple order form, just the presentation layer.
However, you need something in the backend to actually process the payment. Do you have a payment process account somewhere (with Paypal or any other companies)? If so, in the action attribute in the form tag, that's where you need to put in the URL. If you want to manipulate the user input first before sending off to the payment process, you will need some kind of server side technology like ColdFusion, ASP, PHP, ...etc.

Be very careful. The site has to be secure or it may be hacked and your customer details stolen. You need professional help to set this up.
You would also come under the Data Protection Act if you are in the UK as holding customer details on computer require this.

Alot of people use PayPal and use the paypal tools (ww.paypal.com) but some PHP scripts can also do things like this too
