Grade out the area for the driveway so it slopes away from house. Then depending on thickness you want, form it up. For driveways its fine to use 2x4's. All your doing is holding the concrete in and giving it a clean edge. So your 2x's need to be straight. Next fill in with sand to come up to the bottom of the 2x's. Make sure to put stakes atleast every 4' to hold the 2x's. Then lay down wire mesh and rebar. Calculate the cubic yards of concete. Take Lenth x Width (in feet ) and multiply by the 0.33" ( which is 4 divided by 12 for thickness of a 2x4 + settlement). Then divide all that by 27 which is one cubic yard. Then add a half yard for extra and order it. Pour it starting in the back and screed it along the way. It will take you atleast 3 or 4 guys to do all this. Next use a bull float to even everything. Then use a power trowel or hand trowel to smooth everything. make sure to put crack joints about every 6 or 8 feet. Its not an easy task for a beginner, if I was you, drive around at some new construction sites and watch them do it and even offer to help for free.

Step 1. Call a construction company and get a bid.
Step 2. Wait for them to come give the bid.
Step 3. If you can afford it - let them do the work.
Step 4. If you cannot afford it, call another company for a bid.
Step 5. Repeat steps 1-5 until you have a new driveway.

First guy is right. If you need step by step instructions it is probably to big a task to handle. If you dig it yourself and form it yourself you will still need somebody who knows how to finsh and work concrete. Even for the smallest driveway I would not start with out about 5 guys. Working concrete is hard work and there is lot of skill and experiance in making ir right.
