Once a girl reaches puberty, the hymen tends to become quite elastic. It is not possible to determine whether a woman uses tampons or not by examining her hymen.[19] Only 43% of women report bleeding the first time they had sex; which means the hymen likely stretched enough that it didn't tear in the other 57% of women.[20] (wikipeida: Hymen)
If a woman is having her period during sex that can also be 'bleeding' during sex.
Finally, if a woman's vagina is particular small or the mans penis is large it could tear the vaginal opening causing bleeding, as well as discomfort.

Some women don't feel the bleeding until they see the sheets. Some are concerned with the pain that accompanies the bleeding. Some may be spotting while they're having sex.
Most are just concerned with their health. Sex should be the best manifestation of love towards a spouse, although it doesn't hurt to know that the person one is marrying is sexually compatible with you.

Bleeding can be scary. Usually you don't bleed during sex, so when it does happen, it worries people, hence the questions.

because there are a lot of uneducated people in the world. Just because you bleed doesn't mean it was bad either. It could mean that your sensitive, they were to big, ect
