That depends. Many foreign-born infants are citizens of another country because their parents, or one of them, already has that citizenship.
Otherwise a person can obtain a new nationality by registration (especially in the UK) because s/he already has a connection with that country, or by naturalization (if s/he is an immigrant with no family connection to the country).
Finally, one may be made a citizen as the result of a special law (Act of Congress, etc.)
In the USA acquisition of nationality is usually be naturalization. But sometimes it is by acquisition of territory: the Treaty of Paris 1898 is the most important example, but there have been others. (Denmark/Virgin Islands; Mexico, etc.)
