It may depend upon what denomination you belong to. More conservative churches may not think it's appropriate (like the one where I grew up--it was a big no-no).
If in doubt, call the pastor or someone involved in the event, and ask. It never hurts.

My dear,
Halloween is a holiday shared at the latter start by Pagans and Christians. As tradition follows, dressing up is for the sole purpose of disguising yourself and fooling the spirits that come back each year into thinking that you're one of them. The church should realize that by asking people to dress up, that there is a possibility that someone may come dressed as something frowned upon. E.I. : A Witch.
If the church really didn't want you or your children coming dressed up as a certain character, they should send you all a list of what costumes are acceptable and which ones are not.
"Witches" aren't all evil. Again, this is a holiday. Take your daughter to the Festival (By the way, Harvest Festival is the correct meaning of Halloween. By hosting this, they are joining in on Pagan traditions.) dressed as a Witch. If anyone says something to you, politely bring up the fact that they couldv'e sent out flyers stating what was appropriate.

If it is held in a church, they normally do not accept witch or devil (could also be translated as Freddy, Jason, or anything promoting violence) costumes and a number of them will make you change into something else. Pretty much anything else is fine though. If she insists on going on as a witch, perhaps it would be better to take her trick or treating to the mall instead. Witch costumes are forbidden because the Bible mentions in many places not to have anything to do with witches, so it only makes sense that they would ban them. If you have a concern and it's too late to do anything, contact whoever at the church is on the planning staff and get their opinion.
I personally don't have an issue with witch costumes as I am not Christian myself (I attended many Halloween/harvest parties at church as a kid though where everyone dressed up) but it's best to err on the side of caution.

Of course it is ok. The celebration of Halloween, basically broken down is to dress up in order to scare away evil "demons". It would be hypocritical to celebrate Halloween and try to determine what should be appropriate at the same time. Regardless of religious beliefs, now days, it is celebrated so that kids can have fun. If anyone still disagrees and feels that some costumes should not be worn, tell them straight out, hey its a pagan ritual, and your still celebrating it.

a lot of this has to do with the fact that in the Christian religion (I can't say anything about other God/Jesus related religions only because I don't know anything about them, but I was Christian myself), witches were/are seen as devil worshippers. anyone that is a witch is automatically said to be on the side of Satan. in other words, witches aren't liked much by very religious people (the kind that has a closed mind to other religions and believe that the way of God and Jesus Christ is the ONLY way). any open-minded human being won't care. and any sane person wouldn't care if a little girl put on a little black dress, some striped tights, a little pointed hat and carried a broom saying she was pretending to be a witch for Halloween. it's not inappropriate per se, it just may not be taken lightly by some people. if your daughter is determined to be a witch, let her. but if you feel that it may not be the way to go, see if she'll go as a fairy princess or a cat (or something else). go luck with whatever you choose to do.

If they are allowing costumes then a witch costume is fine. There's nothing inappropriate about it. If they didn't want to run the chance of having someone dressed as a witch then they would have said as much.

probably not a good idea to go as a witch, or the devil, or anything that could be considered demonic. My church is more conservative than most. i would use a little caution. If you really don't know, call the pastor or a member of the church who has been going there for a while. It is wise to ere on the side of caution. Whatever you do, have a safe and fun halloween

It depend on the climate in your church. Since they are having a Halloween Costume party, which has pagan origins, it would be hypocritical to restrict the type of costumes the kids could wear.

I dont see why not.. if someone gets offended are they going to kick you out? It's a friggin Halloween party, dang. Tell them to get their heads out of their rears if they have a problem with someone dressing as a witch to a costume party.
If they do have a problem maybe suggest that a list of inappropriate attire be available before-hand next time.
