are there legalities involved in where this information is placed?

Usually patient information and correspondence go on the left side of the chart. Then usually notes, labs, and x-rays go on the right side of the chart. I am not aware of any legalities about where information goes as long as the patient's information is kept together and confidential it should be fine.

Most patient charts are divided up with areas for correspondence, doctors notes, labs, x-rays etc... Legalities? Information should be placed in the correct sections.
