It is called a poll. They go out and ask several thousand people randomly whether they approve or disapprove of the President's actions/policies in several areas, and then average the numbers. The news usually only reports the average of everything. But there are additional numbers available for the individual questions. Those may be higher or lower than the average. Such as if he gets a 50% on the economy and a 10% on foreign policy, this rating would be reported as 30%. Which really does not tell you the whole picture.
A president's approval ratings could be manipulated by simply asking the questions in a slanted method, or not asking questions that would get you answers you do not want to have to average in. (Do not ask about foreign policy, and the President as a 50% approval rating)

The NY Times conducts an in-depth poll.
By in-depth, that means they don't just ask 100 of their readers what they think of the President, it means they ask 100 of their readers what they think the President's nationwide approval rating is.
It's very scientific.

A poll is taken in various areas of the country...then they make their own implications based on the information. The data is purely based on the people answering the questions pollsters have. Neither the questions nor answers need to be factual or fact driven in nature- it's just an opinion.
