I didn't like the hotel I stayed in when I was in Tokyo (Sunshine Prince). But in Kyoto, I would highly recommend Hearton Hotel. It's one of the best hotels I've ever been in. It may seem small compared to American hotels, but I think it sports a classy feel, for a very good price ($100 a night). The continental breakfast is the best I've had of the four hotels I stayed in across Japan. The rooms are Western style, so it feels appropriate, and the rooms are also very good. A big kicker for me was the three computers free of charge for use. Being able to acess the internet was great, although there were times of waiting (other foreigners hogged the computers for hours) but if you are staying for five days, you'll quickly find out the times you'll have access.
I guess every hotel offers this (sorry, my trp to Japan was one of my first hotel experiences), but for some reason, I really liked the safety deposit box that they provide. They have a postal service. And the whole feel of the hotel was just very welcoming. While the other hotels I stayed in felt too cramped, this hotel provided a sense of relief.
Now as I look back to one of your details, I don't know how this hotel rates (I think I read in Fodor's it was 2-star). But for the price, it worked out. The higher star hotels raise the price tremendously (upwards of $300). So it's up to you. There's also one Hearton Hotel (Higashi Shinagawa) in Tokyo, so be sure to consider this one, too. For Tokyo 4 and 5 star hotels, you can go with any of the major international brands such as Hyatt, Hilton, Mariott or a Crowne Resorts hotel.

I recommend the Shinagawa Prince Hotel. It's right across the street from the train station (JR Rail). Close to Ginza, Roppongi, and Shinjuku. Lots of restaurants/bars nearby.
In Kyoto, if I remember correctly, the New Miyako Hotel is good.

I forgot the call of the lodge, yet frequently the nearer(and subsequently greater handy) a lodge is to a station, the pricier it gets. there's a lodge top outdoors Kyoto station(right away in front) that's hooked as much as the in demand Kyoto Tower. From there it takes in basic terms a million a million/2 minutes to get to the station.

I was in both cities 2 weeks ago. I stayed at youth hostels
Tokyo International youth hostel (Your room might be greeted by a compleate stranger as you room mate)
K's House. Kyoto 5 mins from Kyoto station by foot, its great. Quiet by 22:00 as the neighbours are right next door and there were major complaints when I was there so Shoosh by 22:00!

In Kyoto try the Kyoto Centery Hotel...it is just a walk away from the Kyoto Tower, and it is real classy :D

I stayed at Keio Plaza in Tokyo (don't know if its above 4 stars or not), and it seemed to be a nice hotel. Although it is in the city center, its close to commercial or government buildings (can't remember correctly), kind of far from shopping areas and restaurant.
