No. It's simply an expression, much like saying "I've talked to her until I'm blue in the face but she just won't listen."
The condition does exist but there is no actual physical color change.

Orgasm denial is a BDSM technique used during consensual play. It can be a part of CBT (C**k and Ball Torture), or simply a part of the basic rules of a D/s relationship between Mistress/Master and submissive/slave. Orgasm denial in active play, with a great deal of stimulation will cause "blue balls", however long term orgasm denial will not. There is no such thing as a build up of seminal fluid over the long term, its simply reabsorbed into the body. Now, psychologically, used as a control for the male submissive/slave, can certainly lead to a feeling of a need to release, and the pain and stress that goes with that urgency. But that's all in the mind, and that is exactly the point.

I don't know if they actually turn blue, but something like that can happen. I had a coworker that actually had to leave work and go to the hospital the pain was so bad. It turned out to be caused by him drinking too many energy drinks though. Let the be a lesson to the genetic males out there!

A. They're NOT actually blue.
B. Guys DON'T really get blue balls.
C. However, boys/men DO have an active fantasy life, and this imaginary condition is part of it

Actually, there is some truth to that. When a man is stimulated for a time, more blood rushes to the area and remains there, causing it to be more of a purple in color. Relief does help, but it is still not proper to use for pressuring someone into sex.
