I understand social anxiety to be forms of shyness, which may be part of your problem.
Perhaps you need to pick one thing on the weekend and show up there. Or mention it to one of your friends and say you'd like to go and as far as you know you haven't done anything lately to get you grounded so would they like to go with you to whatever.
Everything you do in life is one step at a time, but especially as you start to go out in the world.
Be less concerned with what 'cool' people think or do and concentrate more on being the authentic real you .

Its possible... researching social anxiety will help. I believe social anxiety ,to a certain extent, is normal behavior however when it prevents a person from leading a happy life or stops them from forming functional relationships with other human beings it should be addressed ASAP.
I have family members who never received help for social anxiety and now they are in their 40s and 50s still very afraid of social situations. They locked themselves away from the world as young adults and to this day display the same behavior. It doesnt get any better without help and work on your part.

labeling yourself as suffering from social anxiety disorder is not going to help you feel better or give you courage to go out. If anything it might things worst. First try to relax. Learn some simple breathing exercises and systematically relax your major muscle groups. You might even consider meditation. Also, try not to let things that happened to you in the past (getting grounded) inhibit what you do today. Yesterday is history, tomorrow is a mystery and today is a gift, thats why they call it the present. So get out there and start unwrapping.

I feel your pain... I had a lot of hate in my life because I was a outcast of my age group. I was picked on, and tormented because I stutter. I would not except my cards that were dealt, and because of that it brought more pain... The sooner you just go with the flow, and except that there are a lot of shallow people, the sooner you will be happy. Don't concern yourself with anyone who won't give you a chance. Just move on your marry way.

Well maybe you got grounded too much. Hopefully your parents didn't abuse you...sounds like it damaged your self-esteem. You probably make up excuses why you can't go out I would bet on that. I used to do that too....never thought I was good looking and yet everyone just the girls were always jealous of me. I always got the boys...but I feel my family life was dysfunctional and gave me low self-esteem I do not think you have social anxiety I think you just have very low self-esteem.

Its more important what you think of yourself ~
Yes, it's very possible you have anxiety. Try changing your pattern of thoughts.
It's better to decipher what you think of others, not take up so much of your time worrying what others think about you ~
