If an adoption takes place, a new birth certificate is issued, showing the adoptive parents as if they were the birth parents, and listing the adopted name of the child. This is true whether it is an adoption by both parents, or a step-parent adoption.
The original birth certificate is kept, but is sealed, and only opened in rare cases in most states. However, at least one state has a law alllowing adoptee's access to their original birth certificates simply by making the request. (Oregon)

I dont think any of the information changes on an ORGINAL Birth Certificate.
The mother and fathers names are on there, even if someone is adopted the info stays the same. The only difference is that there is an adoption certificate.
You will have to check with your states law concerning such, and as well as Federal Law. You will have to be more specific though in order to find out what you are wanting to know.

Yes, The court can order changes such as last names, legal father and etc. Some reasons for change would be paternity testing proving the father is other than stated on the birth certificate.When a legal court adoption is made (at least in our state) The court will allow the birth certificate to be changed. OK a little more specific my daughter was born when I was married but it was not my husbands child so when she was born by law he had to be on her birth certificate because in our state they make the husband the father no matter what until proven otherwise. so after proof I had to pay 15.00 to change the father on her birth certificate. By the way our state will also not grant a divorce to a couple when the women is pregnant!

Yes they can and will be changed once the person is adopted. The original will be sealed and from then on the only birth certificate you will be able to obtain is the adopted one.
