Usually you can get this from Human Resources. This used to be known as the Personnel Department, in most major companies. You can also check your employees handbook, or a policy and procedures manual.

i would ask a manager or something but if you feel uncomfortable asking in person you can prob'ly use e-mail or something anonmous (sp?) ly
