I've been trying to figure this out and the first idea was that only Brian could hear him. Then I was thinking about it some more and why wouldn't they be able to hear Stewie, they hear Brian right? So now I'm still confused, I'm just going to go with that they can hear both of them, yeah that's right, they can hear the talking dog AND and the homicidal talking baby, but no one other than Chris can see the angry monkey?????? I don't get it, it's funny as hell though.

It changes. Sometimes everyone can hear him, including his parents and siblings as well as complete strangers, but other times, no-one apart from Brian can understand him. I hope the following webpage will give you some idea of what is the writers reason of doing that:

To me it does not matter. That show cracks me up big time, and I have been watching TV for 45 years. I have taped the show and watched it the next day, and I had to pause the tape a few times cuz my eyes were full of tears from laughing so hard.

they kind of leave that open. Sometimes they reply directly to what he says, and other times they ignore him completely. Brian clearly understands him though. They even ended an episode (The one with Petoria) with the line "so...can they hear the baby, or what?"

Well stewie will say something and
they either aren't paying attention.

Or he pulls out a rifle and lois says " time for bed "

Sometimes they do respond....I think they can understand him, but they just ignore him. Let's face it....the combined IQ of those characters is probably 12

I always assume they just hear baby noises, apart from Brian. But then they can understand Brian....? "Oh my God, you can talk!" LOL
