Dirt gets everywhere. one of the fan blades may be hitting something. Get a can of that pressurized "air". Open the cover and blow the dust , cobwebs and dirt off of everything you can. Make sure all fans that are plugged in are turning. If it is a bearing, it is easy to replace the fan.
Your hard drive may be dieing. If it is get a new one $50 and transfer everything before it is too late. They are easy to replace, just follow the directions. Be sure to get one in the box with instructions and hardware. Don't buy from someone who builds computers. They won't give you everything.

It may be that it's full of dust and getting stuck. Have a look around at the fan at the back of the computer to see if its clogged up - I'm assuming you mean the power supply fan not the Processor fan

If you think its the processor fan then It's very important to make sure it is spinning correctly, otherwise the processor could overheat and be damaged.

When you turn it on, look at the power supply fan from the back of the computer and make sure it is spinning correctly, if that's fine then you need to open up the left side of the case and watch the processor fan when you start the computer, make sure the are no wires hanging loose inside the computer that are obstructing the fan

It sounds like your fan has a lot of dust and debris on it. You can try to use a can of compressed air to remove the dust and debris. If that should fail then it maybe time to replace it.

most likely dirt in the fan bearing.

replace fan. they are cheap.

be sure the fan inside is turning well. if the fan grinds (literally) to a halt, the overheating can kill your CPU chip.
