Could be a vacuum leak. It could be bad transmission fluid (Have it flushed completely not just a pan drop and replace the fluid) it could also be a fuel related issue, clogged fuel filter or weak fuel pump, bad or cloged air filter. The other possibiltiy is there is blow by on the motor (Low compression with the pistions) try some engine restore and see if that helps maybe even a heavy grade oil.

First...It has nothing to do with the gearbox. Second...Things like hills put a higher load on the engine, as you're not only trying to accelerate the mass of the vehicle, but you are also trying to overcome gravity. There's 2 systems that are affected the most and can cause an issue like you are having under those conditions. The first, like the previous people suggested, is the fuel system. A clogged or partially clogged fuel filter can manifest itself with the exact symptoms you described. A weak fuel pump, if electric, can also have similar effects. The second system is the electrical system. If the spark plugs are partially fouled, or a burnt plug wire, you might not notice it during normal driving, but on that hill the problems will show up. The first thing I'd do, is a filter change. Change the air filter and the fuel filter. The fuel filter is often forgotten on tune ups by average people, but it is supposed to be changed regularly. A fuel filter is typically anywhere from $5-15, and an air filter $5-20. Then I would check the spark plugs and wires, because they also should be changed on certain intervals, and are the 2nd most likely cause. Spark plugs could be anywhere from $8-30, and the wires are another $25-50. If those don't work, finish the tune up with a new distributor cap and rotor, possibly even a new coil. Only if those don't work, do you go further and check things like the fuel pump.

The spark plugs don't give that failure.
1º Check your ail filter
2º Check fluid transmission and filters maybe need change
3º Check if gas is ok maybe the throttle body is not in good condition and when you give gas it is not ok
4º Your electric system is not working correctly and don't give good timing ignition resource and you loos power
5º Are you using gasoline whit more octanes than the car require? be carefull cars are designed for a specific octane if you put gas with more than required the engine ignition will be retarded, you will not win more power u will loos power you must use that the car need no more no less.

I think it would be cause it is tired. It is old so this is to be expected unless you rebuild the engine. And since you are out east you have bad winters and that means salt which means rust so unless the body is in good shape. That seems to be your options.

Check you transmission fluid. If it is low, it may cause the car to downshift when it gets into the slightest pull.

sounds like a vacuum line is cracked or dried and cracked or broken, hence Vacuum leak,
