My wife has a reoccuring dream that I have sex with her sister, what does that mean?

She may be losing trust in herself or you. She probably is in deep anxiety and stressed out.
Rather than projecting someone other girl, she chose her sister because deep down she does not want to lose you to some girl out of the family. She is reserving something for her ownself. Subconsciously she thinks only family remember can deserve that kind of relationship.
By projecting her sister to have relationship with you, that means there is some obstacle in relationship between you two that you must resolve.

Dreams are subconcious wish-fulfillment. But that doesn't necessarily mean she wants you to bed her sister. Sometimes the fear of what is possible is worse than the actual event. If she's so afraid of infidelity that the fear is worse than the act, her subconcious will "create" the act to "cure" the fear.
It's an insecurity thing with her. She picked her sister because she's been both her rival and her support all her life.
