Yes, you should have a doctor's supervision when using any fertility treatments. They have to monitor your dose and make sure the medication did in fact help you ovulate. Depending on whether or not you get your period after trying clomid, the doctor may wish to induce your period and test to make sure the clomid did not leave any ovarian cysts or up your dose to a higher level to help you along. You should also make sure you are taking prenatal vitamins if you are trying to conceive.

Yes because it can cause hyper-stimulation in your ovaries. When you are on Clomid, your Dr. will have to either examine you or do an ultrasound to check for this. There are generic equivalents to Clomid and you only take it for 5 days, so the cost is not that expensive. I get it at Costco (which you don't need a membership for prescriptions) and I only pay $15 for it.

Yes, and they have to give it as a prescription. They start you out on a low dosage of 50 mg. Then between days 24-26 you go back to your dr and they test your progesterone in your blood to see if you ovulated. If you did ovulate then you wait until your 30th day for your period. If you get your period then they will up your dosage to probably about 100 mg. Then you will need to get your blood tested again days 24-26. I have to go to my dr's tomorrow for my progesterone test.
