My favorite in the whole world is "The Blue Elephant" Thai restaurant in London, near Fulham-Broadway tube station. Outstanding.
Second favorite is "Devant des PTT", a cozy teppanyaki place in Tokyo's Roppongi district.
In the US...it would have to be Morton's steak house in San Diego.

French Laundry in Napa Valley. I believe they're the first restaurant to get the best rating in the new American Michelin guide.
If you can handle knowing that the owner is Thomas Keller. *rolls eyes*

There is a chain of Italian restaurants that I LOVE LOVE LOVE called Bravo! Cucina Italiana. They are moderately priced, a very classy atmosphere, and great service. I don't know where they are located in your area (I don't even know where you live), but there is one in Kansas City, MO that is amazing!

depends where you live. most "chain" restaurants won't be 4 or 5 star restaurants.

Morton's Steakhouse. It's pretty expensive, but the steak melts in your mouth and the service is WONDERFUL.
