First of all, you need to take an equal amount of magnesium to your calcium dosage. Your body needs magnesium in order to absorb and utilize the calcium. Vitamin D DOES also play a role in absorbtion and utilization, but it's further down on the list from the magnesium.

Vitamin D dosages are predicated on different factors, so check out: http://www.nof.org/prevention/calcium.htm

400 IU (International Units) of Vitamin D per Day has been found to be most effective to aid the uptake of Calcium.

If you take Calcium with meals that will improve the absorption rather than on empty stomach. The clinical study on Citrate Malate was done on an empty stomach to compare the absorption with Calcium Carbonate. However, when the study was repeated with full stomach no difference in the absorption was observed.

NOTE: In the previous post someone has mentioned 250 mg but that will be a TOXIC Dose for Vitamin D.
