Bandwidth is what the communications link for the server is offering. It is separate from the server. Unless you have a commercial site that needs large amounts of bandwidth you should be able to find a comm link (like a local cable company) that doesn't charge extra for a lot of external bandwidth.
Of course if you are taling bandwidth to serve computers in your own home or office then bandwidth just depends on what kind of cable you installed. There is no ongoing extra cost

Because there is no such thing as a free lunch or anything else. Everybody who had a thumb in the server pie gets their cut. When I see offers that say "free" I run like the wind because it will probably cost me more than the companies that come right out and tell you costs upfront.

Same reason as why when you buy a computer, you have to pay for an internet connection.
The two things are made, sold and run by different companies.
