It means that it's not something to get upset about. In other words it's so insignificant that it won't make a difference. Like if you're cooking, a grain (or granule) of salt isn't going to make or break the meal, you'll never notice it.

If a doctor said it to you it was probably meant literally: you should take your medicine with a grain of salt. And possibly a glass of water.
