Since ‘item’ is singular, it is my spectacles (pair of lenses in a frame resting on the nose and ears, used to correct defective eyesight).
However, I do not leave home without the following:
1. Purse (Handbag / Money bag).
2. Driving Licence
3. Spectacles
4. Pen
5. Mobile Phone (Cell Phone)
In fact, I have to count five items prior to leaving home for work as I am now aging and keep on forgetting things (LOL). So, count of 5 makes my watch list complete and these are most important items in my life.
