I honestly think that these are normal fears. Doctors can very easily make some people nervous, and other, like youself, scared of them. Maybe you should talk to someone close to you who knows you well and ask for their opinion, just because they mae know you better than anyone here.
As for your fear of death, I would say almost every sane person has thought about death and in some way or form has been scared / anxious. Because we as people do not know what happens when we die, is a natural response to facing death. So as for overcoming you fear of death, I would say find something to live for. A relative, anything that makes you WANT to live. I dont know if this will help you any, but I hope it does. =]

However you should not fear doctors because they are professionals trained to take care of specific needs for people. Those are pretty common fears, and everybody is afraid of something or another. I hate spiders. A common method of treating phobias is called systematic desensitization. Starting really slowly, a person is exposed to what they fear, and they practice relaxation techniques to stay in control. In just a few months, a person can handle easily what used to scare the heck out of them. If this sounds like something you want to do, try a mental health association in your area. They'll know what services are available to help you. Good luck!

Honestly, it is hard. First off, make an appointment. Try to get one with a female doctor. Sometimes too it is easier to see a female PA, they can sometimes be more understanding. Right off the bat let them know that you have fears and they may be able to help you or at least make you feel more comfortable. Good luck!

Facing them is one step. You need to ascertain the root of your fears like a bad experience at the MD's office a long time ago. BTW, I am afraid of death myself because I want to see my son grow into a man.
