That depends upon which version you are speaking of.
In the original Dead series done by George A. Romero, the outbreak is sort of explined in Night of the Living Dead (1968) as some form of radiation from Pluto.
One character in Dawn of the Dead has his own ideas: "When there's no more room in Hell, the dead will walk the Earth."

the exact cause is never fully explained in dawn of the dead, but in night of the living dead (the prequel) they explain that it is caused by some sort of cosmic radiation brought back by a space probe

a crazy person in a bar bit a guy and then it all started. they should of had a better plot planned because i love that movie but the begining didnt explain in detail how the crazy person got it.
