birth control i.e. the pill, they can probably tell by seeing certain elevated hormones. but they'd have to be looking for that. I don't know which tests would pick that up -- there are lots different blood tests which look for different things. Not sure if the standard checkup blood tests do.

a blood test can see if there is anything including birth control in your blood.
