Self-awareness is just being aware of yourself, your needs, wants, things you want to fulfill. I think it's like the top of the pyramid scheme of needs.
Self evaluation is looking at yourself and finding out how you've been going along with everything, how you deal with certain emotions and situations.

Self awareness for me is being aware of all the things i do, i think, and say. In the moment. Not as analyses or retrospection.
Self evaluation is when im not aware and am judging my self and my actions against the conditioned response's which are deemed right or wrong.
Evaluation is measuring up to.........comparison, imitation, fear, or simply to evaluate exactly what we mean by that word self.
It all depends on the direction you want to pursue.

Self evaluation is more of a judgement of how you think you are going, is what I am doing right or wrong as a measurement. Evaluation is to measure against something.
Self awareness is a conscious knowing of consciousness. No judgement, just awareness of Self. This term is usually used in a spiritual context. In the attainment of states of enlightenment for instance a high level of self awareness is attained.

self-awareness: being aware of your own individuality. You know who you are, where your going, and what you're going to do with your life.
self-evaluation: usually involves the evaluation process by writing self reflective essays (which is usually) on what one is learning. Students have a chance to evaluate their effort on projects by writing self-evaluations, making goals for the future and using the class created for quality work. When students have a chance to reflect, Their teachers/professors learn a lot about what they think about their work and work habits. The students learn to judge the quality and amount of effort they have put into a piece of work. They also learn how to make goals for themselves.
Some write essay form self-evaluations; others write in simply list-form. It would be wise to ask your teacher/professor how s/he'd like you to do yours before you start.

Self awareness is a state of knowledge. Self-evaluation is a process through which you can develop that aforesaid state of knowledge. To evaluate is to consider the value of - therefore self-evaluation is to consider the value of yourself, i.e. to think about your strengths and weaknesses, compare yourself to others, judge yourself and so on. I would think that to be self-aware you need to engage in some form of self-evaluation.

Awareness is the broader term: paying attention to what's going on with you.
Evaluation means judging, assessing, saying 'good' or 'bad' to.
So evaluation presupposes awareness.
(Take out the 'self' part, and maybe it's clearer. If I'm aware of the carpet, I'm simply taking note of its existence; if I evaluate it, I'm saying whether it's a good or bad carpet.)
