Right after the huge party where Tom Cruise was really drunk, he collasped in an alley. His life then became worse and worse until he decided to commit suicide. Since he saw that commercial on tv that said one could be frozen and brought back to life... before he died he signed a contract to be frozen after he died. Also, he chose the lucid dream aspect of this frozen state which allowed them to implant an alternate life into his brain, erasing all the real life memories. The split between the real events and the alternate events happened that night after the party which I have mentioned in the first sentence. So, he would have all of his real life memories before the party but new ones after the party.
Though, a malfunction happened in his frozen state in which his mind started creating mulitple variations of events (including some of his real life memories) and combining them all into one. This caused Tom Cruise's character to feel insane. Then he saw that commercial that led him to sign that contract to be frozen and that triggered some of his real life memories that occured after the party. This caused him to seek out this company and there, he encountered the man that told him it was all a dream and told him everything he didn't remember about his real life.
Since, he was in the future and they had the technology to repair his body... they gave the Tom Cruise character a choice to take a new life or to start the dream over again. The Tom Cruise character eventually chose to wake up and live out the rest of his life awake.

I never got the impression he died at all, rather he was just frozen, like the dog.
But yes, what a couple of people say about his "fantasy" life starting after he leaves the nightclub is right. From what i gather, he still felt responsible for Diaz's characters death, that his psyche created a nightmare, rather then the "lucid dream"
He is given the choice of going back into the "lucid dream" where he would have a happy, all be it, artificial life, or end the dream, and be "re-animated" and re-enter the real world.
He decides to come back to the real world, says his goodbyes to Cruz's character, conquers his fear of heights, jumps off the building in order to end the dream, and fade to credits, assuming that his dream is now oever.
Fantastic movie though =)

That movie was awful i regret watching it. but basically all the movie from the point he walks out of the party is a fantasy he signed up for that program and it satrated to go bad.so they let him out in the end which was like years and years in the future he found out he had to jump of the build ing becase he was afraid of heights and that was the only way to wake up from the dream world that was haunting him.
