In my experience, DSL bridges ("modems") don't have usernames and passwords of their own. So I have to guess that you're either trying to change the admin username and password in a router, or trying to change the info on your (or someone else's) account.
If it's a router you're trying to reset, how you go about it would depend on the make and model. I'd suggest visiting the manufacturer's website and checking for documentation.
If it's the account itself, you need to talk to your ISP about that.
If it is in fact a somewhat fancier DSL bridge -- perhaps one of the commercial ones which sometimes do incorporate routers and other networking hardware -- you're definitely going to have to read the documentation and/or contact the manufacturer's tech support department.

Usually the userid and password are stored as part of the PPoE client on the PC or router, not in the modem itself.

You can't change by your own. You have to contact your service provider of your ADSL.
