A subscription to "Click" or some other children's magazine; keeps the fun coming all through the year and is educational. I loved getting things in the mail addressed to me when I was a kid.

If they have everything then I would suggest an experience is better than an object. How about a trip to a pantomime. It is very festive and would be a wonderful day out for you both. You could get them a little costume which relates to the story i.e. Peter Pan and you could get a boy a pirates outfit or a girl a tinkerbell outfit!!

A four year old that has everything sounds a little sad to me, maybe something like a trip out, they do day trips to Lapland etc (costs the earth tho) a personalised letter from Santa can be bought from ebay as well.

Books, money in a bank account (existing or new) OR best of all have him wrap some of his own toys to give to a less fortunate little boy( his age) for Christmas. Family services will help you with this distribution.
