Depends how big of a spot it is. A no fail way is with hydrogen peroxide. Put a towel under the comforter under the stain and pour a little peroxide on the stain and it will bubble and foam. Use another towel to blot it up and keep doing this until the stain is gone. I got blood on my new mattress (you can guess how if you are a woman) and I got it all out doing this. It takes some time, but it gets it all out. And unlike bleach, which just turns it white, the peroxide really gets it out, and has no smell. Try it. You will be amazed. My sister had a spot of blood on a white shirt and I told her to do this and she did not believe it until she tried it and the blood was gone. Will work with old stains too.

Put detergent directly on the stain with NO water and let it soak for 24-48 hours. The soap will dissolve the blood and will be completely gone after a thorough washing.

I have used that OXI clean in the past on cherry kool aid and red wine on a white carpet and you can never tell there was anything there. It does say it works for blood stains as well. Good Luck, its worth a try.

Coca cola also breaks down blood. Soak the spot in Coke for awhile and then wash as usual with detergent.

I would not do it myself. Dry clean is the best way to go I think. I remember I tried to clean a 100% wool comforter/blanket and now is totally ruined.
