Accents never annoy me, provided the speaker is speaking in a polite tone. It really depends on the heart/mind of the person who is speaking. A ' bad ' accent can be overcome rather nicely if expressed by a polite person.

Geordies, mostly because of the knuckle dragging moron thats generally behind the accent. Geordies are the ultimate example of the 'im great because of where im from mentality' all because Jimmy Nail said bollocks a lot on TV and had a few minor record sucesses in the 80's, oh and because they spawned a drunken wife beating idiot footballer.

It used to be South African, because it just makes people sound so bloody arrogant!!!!! That was until I met my b/f, who happens to be one of a minority that does not come across as totally up themselves. Sorry, no offence meant there at all, my mother is also South African, but I have not met many friendly ones. Also, the Brummy accent is not the most pleasant.

Middlesbrough.
Those whiney aberdeen accents are pretty hellish, too.
Further afield, I guess Texas and Lousiana accents sound ignorant and annoying.

I'm a scouser and hate hearing it on films when it is portryed like a thick accent like Harry Enfield 'Calm down, calm down' scousers. I think geordie is sexy and N Ireland.

I dont think foreign accents bug me, but certain Northern accents in the UK sound like awful mutilations of the language.
Phrases like 'It were brilliant!' spring to mind.
