I'm unlclear as to whether you're looking for window panes (just the glass), or if you need a replacement for the whole frames and panes assembly.
If you just need a window pane, Home Depot or a local glass shop can help you out. You'll need to know the exact size of the original pane, though, which can be tricky. If you can get another pane out without breaking it, you'll be all set. If not, stick a ruler into the slot where the window pane used to go. You need to measure how much of the pane is hidden by the window frame on each side. Usually this will be 1/8 to 1/4 of an inch. Multiply this number by two and write it down. Next, measure the height and witdth of the visible pane. Add the number you wrote down earlier to both measurements, and you should be good to go.
However, rather than mess around with panes of glass, here's another option: Go to Home Depot or Lowe's and find out where they sell their plastic sheeting. You can buy sheet platic (plexiglass) in several different varieties. The best one to get is Lexan (or polycarbonate if generic), which is 500x stronger than glass, is just as clear as glass, and insulates your house better than glass. It's much easier to cut and work with, also. An average sized Lexan window pane will only cost you about seven dollars, and the store selling it to you will cut it for free, whereas you usually have to pay extra to have your glass cut.
If you need the whole window assembly replaced (frames and all), then you're in for some hassle and expense. You're going to need to take the old window to a contractor or a specialty store. Some Home Depots contract with outside companies that provide this service, but don't expect it to be cheap. Most companies will charge $200 or more for a replacement window, depending on its size and complexity.

If you are looking in a Home center then you will only find "stock sizes" which normally are not the size you need.You will have to order what you need.I would suggest if you are going install them your self that you measure the "box". This is the inside of window area ("the box") to with in 1/8 of an inch.You need to measure the width and height. The size is expressed with the height as the first figure. Once you have the measurements , go to the the home center give them the sizes and state that the measurements are "tip to tip". Decide before you go in which style of window you want.e.g. casement, double hung.The process of installing is as follows:remove the trim inside then remove the "stops"(these are the wood pieces that hold the sashes in place).Remove the sashes, weights, ropes or chains.Do not remove the outer stops. You now have an empty "box". Read the instructions that are supplied with the new window.Dry fit your new window.Once you see that it fits properly, remove the new window and caulk the inside of the outer stop. Install your new window and press firmly in to the caulk.Once you have made the adjustments according to instructions,you will caulk the window between the window and the box. Now you can re install the trim that you removed previously.Note: you may have to install a stop at the bottom of the window on the outside at the sill.

Harvey makes great replacement windows and they offer a free low-e upgrade to homeowners when you buy replacement windows. (It's usually 30.00 or so a window!)
