Eventually, the pressure inside your bladder would become so great that you would experience reflux, a condition where urine travels back through the ureters into the kidneys. A healthy person has valves at the ends of the ureters that prevents reflux, and surgery is required to correct a deformity of those valves. Reflux induced by pressure would probably not require surgery to correct, but would still result in the dangerous introduction of toxins and bacteria into the kidneys. A single incident would probably be painful, due to the bladder pressure, but would not cause significant or lasting damage. Really, you should just pee. And it's possible that you'd lose control over your bladder and just wet yourself before you managed to induce reflux.

Well, you got several good answers. Did you know that a burst bladder and resultant infection from "holding it in" is what was once thought to have killed the famous astronomer Tycho Brahe (though current research suggests this not to be the case).
Yes, "holding it" can lead to infection(female nurses are a group known to get UTI's from the inability to get away from the bedside to void at frequent enough intervals), burst bladder, hydronephrosis, and eventually to renal failure and death. Though, as others have said, you are as likely to pee yourself before suffering the worst of these things.

the pressure in your bladder continually builds up. it sends signals to ur brain. finally u will have to let go. it is anormal human response. beware. buildup of urine causes buildup of toxins and may lead to kidney stone formation and urinary tract infection
