It's perfectly correct if the person you're talking to has a message to leave. But, "May I take a message?" is a more normal manner of asking.

the word "may" is a polite way of asking a person. the message "may i take your message" is right depending on the qoutation of a person needs, and "may i take a message" with a conotation of "sir/madam" is definitely with ethics of politeness.

I am not sure within what context your question would be but usually one asks:

" Would you like to leave a message?"
