If the service is adequate, then you should leave 15% of the bill (NOT including tax).
If you receive excellent service (above and beyond the call of duty), then leave somewhere between 20-25% if you can afford to do so.
Attitude and politeness counts for a lot, but I also tip on how well the server reads the situation (i.e. you're on a date and want to have an intimate conversation without a lot of interruption-if your server reads the situation correctly, he/she won't be around all that often, but they will be there when needed. You may think that they're not giving you good service, but in actuality, they are giving you what you probably desire/want. TIP them accordingly).

depending how much your meal cost. If server gave great service I leave 20-30% tips off your bill. If server was okay I just give 15%. server is terrible leave just 10%. If people have money to come out and eat I believe they have $$ to tip at least 15-20% if not just call take out and serve yourself at home. It's much cheaper.
If you're a bad tipper don't even come back to the restaurant. You don't wanna know what they will do to your food the next time you come back.

20 to 25% for good to great service. Okay, I know that's high, but if the service is good I'd leave 20%---and if the server contributed to a fabulous time that was had by all, I'd leave 22 to 25%. And if the service was not so hot, I'd leave 15%. I am capable of leaving nothing if the server was hostile but it would have to be an extreme. These folks get paid minimally and they are working to support their families. Again, I know I'm leaning towards the high side.

It is NEVER good form to punish the wait staff for conditions obviously beyond their control, e.g. lack of certain food or beverages. Wait staff are paid a tiny amount per hour, and depend on tips to make a living.
15% is the polite minimum. For great service, feel free to go up from there.
My husband and I would like to invent a Tip-O-Meter. The meter would start at 15%, and then every time I get a refill without having to ask for it, or my kid gets his cheeseburger without pickles, the tip goes up towards 20%. If I have to ask twice for steak sauce, it starts dipping back down again. Then they don't have any surprises when they get their tip, and they have incentive to do better! LOL!!!
We are generous tippers and we almost always get good service. There are too many rude people in this world who leave little/no tip. If a person cannot afford to leave a tip, they need to eat at McDonald's.

I always try to tip about 15%, even for just 'okay' service. But if the service sucked, I'll leave 10%. I think I've walked out without tipping only once or twice.

Assuming the service was satisfactory then here's a quick way to figure out how much tip to leave:
For breakfast or lunch, customary is 15% of the BEFORE TAX amount. To quickly calculate this, you can usually just double the tax amount (for example, if you live in Ventura County, the tax rate is 7.25% so double this amount is 14.5%; if in parts of LA County, then the tax rate is usually 8.25% so double is 16.5%).
For dinner, 20% is customary.
If buffet or self-serve, then 10% is adequate.

I always leave AT LEAST 15%, even if the service wasn't that great. If I receive great service, I tend to leave anywhere from 20-25%, which is not much considering I don't have to cook or clean up afterwards =)

If horrible service then please 5%
If okay service leave 15%
If excellent service leave 20%
Remeber, its not the waiters fault if something is wrong with the food and dont be shy to tell them if u dont like it becuz the chef can change it. (and sometimes service is slow becuz either the restuant owner doesn't want to get enough employees or its just busy. Im a a waiter and im just saying that i think. Just they say u dont know anything about that person until u walk int heir shoes)
