If u rephrase ur question: "can a circle and 4 chords of the circle divide a plane into 12 regions?", then the answer is yes.

2 chords can create 4 regions within the circle and there is the region outside the circle so that's 5.

A 3rd chord can create 3 new regions by intersecting the 1st two chords at 2 distinct points (1 per chord). So 5+3 = 8

The 4th chord can create 4 new regions by intersecting the first 3 chords at 3 distinct points. So 8+4 = 12

However the circle itself will consist of 11 regions. The outside region is the 12th

3 straight lines horizontal and then one U-shaped line with the top off the outside of the circle with the straight parts perpendicular to the horizontal ones.

(does the question specify straight lines?)

A "U" shape is not a line! That won't work. You could then just draw a big squiggle that crosses itself.
