smoke or steam...steam is the heater core usually ...smoke...well...where there's smoke there's fire.
i would start with the heater core-especially if the floorboard on the passenger side is wet.

Heater cores don't smoke, they leak.
Smoke and defrost are two separate problens.
For smoke problem, roll down window, should help defrost problem also. Wear warm clothes if cold.
Now tell us, does the smoke have an odor ??
Does the heater fan work ??
Does the heater heat ??
Is the floor wet below your heater ??
