I guess I don't have to tell you that it stands for "also known as", but since nobody seems to care about the second part of your question (seems like they're more interested in getting two points to tell you the same thing!), I thought I'd help you out.
The origins of "a.k.a" are, according to my copy of the Oxford English Dictionary, American. Unfortunately it does not give me any more information.
Because it probably did not appear in print very often (a.k.a would probably have been written as "also known as"), there are no records of its use. It probably appeared over a number of years probably no more than 100 years ago and slowly made its way into the vernacular. That's the best I can do!

I remember hearing one time that "aka" was used as an abbreviation in the US military for "also known as " on their telegraph type machines. It saved time when sending a message and to get the message to the person. (same reason Americans spell many words without a "u" in them as compared to UK english; eg "colour" vs "color", "favourite" vs "favorite" etc) Later telegraph machine operators charged per letter in a telegraph so "aka" was used then too.
