people do it all the time with plastic surgery...
in a relationship though, if it needs to be fixed, then something is broken.

No! One event must come first for the second event to be able to happen. For example, a ball can't fall in the ground without being thrown or let go. Even if you take the ball and hold it, then you hit the ground with it is still a fall. You can improve something but then it would not be that you fixed it but you went over the objects neutral state.

Depends on what you're talking about, but I suppose it could still be fixed if it isn't broken. Not until it is perfect can it not be fixed. And what do you know of that's perfect?

yes. everything can be fixed or make it better. but general rule of thumb is, if something is not broken (or working) don't toggle with it and try to fix it.
