During the opening sequence, the lips are those of Patricia Quinn; the singing voice is Richard O'Brien's.

Those are Patricia Quinn's lips. She was really excited about singing the opening song but the production company decided to go with someone else. As the story goes she was sitting in a car with Richard O'Brien and Tim Curry

Tim: "So Pat, if you aren't singing Science Fiction Double feature, who is?"
Richard: "I am"
Pat: "You BASTARD!"

She was very bitter about that. Now there was a whole bunch more to his story about where the concept of using lips for the show came from, but once they decided on that someone approched pat about portraying the lips that would sing the song in the movie. She wore all black makeup, was put into a clamp type device to keep her head still, and she was wearing caps on her teeth (you can see them if you watch really closely).

The lips on the poster were not hers however.

It is Patricia Quinn... aslo to add to the knowledge... her agent didn't want her to take the part because she only got 4 lines in the entire film, but the trade off was that she got to be the lips in the opening sequence, that's why she took the part. :)
