I have carefirst (blue cross/ blue shield) PPO. I love that I can go to a hospital anywhere with minimal to no co-pay, but I don"t like the high cost. I also don't like that, if I am injuried on the job, my insurance will not pay for doctor's care even if I want a second opinion (IWIF doctors are not always looking out for your best health interest, they just want to certify you back to work). I am a state employee and I am also very dissatified with my prescription co-pay. A true arm and leg.

I'm very satisfied with my insurance. My copay is only about $20 and I also have dental and vision insurance covered by my employer. My primary doctor is also included in my plan so the deductible isn't as much.

speaking from experience, not many. some policies exclude some things but not others. I work for a health insurance company and there are many different policy types.

I work where they just changed carriers and our new insurance is not as good as the old one was. Still pretty good for the money though.
