I got up and walked out of The Patriot about 1/3 of the way through. All those carefully-filmed chunks of flying gore made me SICK.

I have seen vids on the Net that were FAR worse. The one of that poor, bruise-covered, God-forsaken woman going down on a HORSE, then puking, probably would be the most sickening, soul-aching thing I have EVER seen in my ENTIRE life.

Audition.... the last 15 minutes.. was pretty gross
Ichi the Killer... really nasty scenes..especially the tongue cutting off scene..
Henry: Portrait of a Serial Killer.. very disturbing..
but.. the most awful..sick..disturbing movie of them all... and it is a real b i t c h to find... is
August Underground ... really really not for the faint of heart..

Slaughtered Vomit Dolls gets an honorable mention.. I've never scene the movie..but.. I couldn't even get through the trailer without feeling a bit ill... so I can only imagine sitting thru the entire film.

In Zombie Flesh Eaters, there is a scene where a women is hiding in a kitchen behind a door and a zombie punches his hand through the door grabbing her hair and pulling her toward a splinter of wood jutting out of the door until it punctured her eye.
This scene gave me a phobia about having anything near my eyes for over a decade. I kept on thinking that my eye would pop.

The scene in Casino where the "even worse guys" beat up the other bad guys and throw then into a pit. Those bruised legs look so horrible! I stopped watching the film...
