urinary tract infection (UTI) is the most common with this type of symptom
other symptoms of a uti are:
-painful urination (burning sensation)
-frequent urination or urgency
-fever (sometimes)
if he has any of these other symptoms, he needs to be seen within 24 hours to get antibiotics to treat the infection. if he chooses not to do this, the infection could spread into his kidneys causing worse infection and pain. kidney infections often require hospitalization
in the meantime-have him drink lots of fluids to ward off dehydration and cranberry juice to help flush out the infection in his bladder.

He could be an avid pot smoker. Other things include an u.t.i. or kidney infection. If he has a stone moving down this could be a cause too. At any rate, blood in the urine should be checked out first thing.

It would be a kidney stone. I have had them ago and that is how I consider while a stone is commencing to beyond and it best will get worse. It will also be a kidney or urinary tract illness. Any of those will must be dealt with by means of a general practitioner. I wish you get to feeling bigger quickly.

Are you referring to blood in the urine only, or about blood in the semen also? I ask this because if a man stays aroused for a long time, it can cause pain like a blow to the testicles would. It can also cause blood to appear in the semen. I don't know if it would be noticed in the urine, though.

It could be a bruised kidney? If he has recently been hit in the nuts, he could bleed temporarily? He should receive medical attention though. That is not a good sign. It shows internal bleeding.

Urinary tract infection. Injury to kidneys from some sort of trauma, like falling off a ladder.
That is all I know of.
